A 27-year-old man named Patrick Liam Coyle, who hailed from Wigan, tragically passed away shortly after reaching out for assistance from medical professionals, an inquiry has revealed. Coyle was discovered deceased at a residence in Accrington just six days following his discharge from a mental health facility. During a two-day investigation at County Hall in Preston, it was disclosed that Coyle had actively sought help in the days leading up to his demise.
After being discharged, Coyle visited the emergency room three days later and implored to be readmitted on March 7. Despite his plea, he was deemed safe to leave as he denied any intentions of self-harm. Unfortunately, no contact was made with him after March 8, and he was found lifeless two days later.
The young man, employed as a warehouse operative, was declared deceased at the scene. The coroner, Kate Bisset, disclosed that Coyle had expressed a wish to return to inpatient care following his recent discharge from a mental health facility. She stated that Coyle had struggled with his mental health for an extended period and had experienced challenges with alcohol addiction that heightened his risk of impulsive actions and self-harm when intoxicated.
Coyle had resorted to excessive alcohol consumption and medication intake in the days subsequent to his discharge. Despite expressing a desire for inpatient care, mental health workers assessed him as safe for release when he visited the emergency department on March 7. Mental health support workers visited him the next day and noted his denial of current suicidal thoughts. Plans were made for ongoing support, but Coyle was not heard from after March 8 and was later discovered deceased on March 10. Due to his intoxicated state and deteriorating mental health prior to the incident, it was deemed challenging to ascertain Coyle’s intentions at the time he engaged in the fatal act.
